Texas HCN and FFS Program
The Prime Health Texas HCN and FFS Program offers participating members with locations in network service areas access to our network of providers. Our network providers are focused on getting injured employees well and back to work.
You must provide the notice of network requirements and the acknowledgment form in English and any other language common to your employees. If you need a copy of the notice and acknowledgment form in Spanish or another language contact Dorrence Stovall at dorrence.stovall@primehealthservices.com.
